2013-10-16 12:25:54 - Intrauterine insemination (IUI) or artificial insemination by husband (AIH) or artificial insemination with donor’s sperm (AID) is a procedure where the doctor is giving nature a helping hand by increasing the chances of the egg and sperm meeting. The goal of IUI is to increase the number of good quality sperm in the prep that will eventually fertilize the egg.
Artificial Insemination is a silver lining for all those couples who are unable to conceive or get pregnant due to various reasons. It is a technique that can help treat certain kinds of infertility. It is a procedure where semen is introduced into the female’s uterus. There are many techniques through which the fertility can be achieved.
Creation is a centre for Infertility Treatments and Assisted Reproduction at Fortis Hospital, Bannerghatta Road that provides comprehensive assessment and evidence based solution for such couples seeking assistance to overcome infertility.
Dr. Manisha Singh, Consultant Gynaecologist and Subspecialist in Reproductive Medicine & Surgery, Fortis Hospitals says that one may not be able to conceive due to many reasons such as male partner’s impaired
semen quality, ejaculatory dysfunction, anatomical defects etc. It may also be the female partner’s tubal dysfunction, minimal to moderate endometriosis or inability to have intercourse etc.
The IUI procedure starts with the assessment of the follicles (cyst like areas which contain eggs in the ovaries) from the second day of the menstrual cycle via ultrasound scan. Stimulation of follicular growth can be achieved by giving hormonal injections or tablets. Follicular scans and blood tests help monitor the growth of the follicles. HCG injection (hormone) has to be given 12-24 hours before the procedure as the final trigger to allow for egg release from the follicle.
On the day of the procedure semen sample collected by the patient’s husband or a frozen donor sperm sample is washed and prepared in Andrology laboratory. The prepared semen sample (0.4ml) is loaded in an IUI catheter and inseminated slowly over a minute to avoid back flow.
After the procedure the patient is asked to lie down for half an hour for the sperms to disperse through the uterine cavity and tubes. Progesterone is given for 2 weeks and continued for 12 weeks if the patient conceives. The patient should refrain from smoking and drinking alcohol during the treatment and ensure that she has a healthy diet.
Mrs. Shanthi, a patient at Creation said “When I first met Dr. Manisha Singh I had already undergone 2 IUI treatment cycles plus other treatments in various hospitals but it was all in vain. We had decided to undergo IVF at Creation and we met Dr. Manisha Singh. After reviewing our case notes she made us realise that IVF was not necessary yet and she would prefer us to have one more go at IUI. She explained that IUI is almost like a natural cycle and less expensive than IVF. We took a chance and underwent the IUI cycle at Creation. The technique followed by the doctor was really different compared to previous IUI’s which was done in other hospitals. After 15 days from IUI to our surprise it was positive.”
Another patient Mrs. RV said “We were trying to conceive from past two years but we were unsuccessful even after trying an IVF + ICSI. We had almost lost our hopes but as one last try we walked into CREATION. I and my husband were extremely happy and positively hopeful from the moment we walked in. Dr.Manisha was a wonderful and pleasing personality to interact with. She kept enforcing "extremely positive" thoughts and feelings all through the interaction. I was apprehensive when Dr. Manisha said you guys are healthy and normal. So I went for a few cycles of IUI. I doubted if it was the right thing to do. We are extremely elated now that I am in the second trimester.
IUI is a cost effective treatment and takes less time as opposed to IVF. The success rate of IUI is between 12- 20%. But few of the risk factors involved in IUI are multiple pregnancies, ovarian hyper stimulation and stress.
